"Right now" is a pretty common English way to say "now" the way "сейчас" conveys, so I'd look into that and make "right now" acceptable wherever possible where "сейчас" is in this course.

I answered "Is the doctor in town" got it wrong even thought it's the same thing as "Is the doctor in town, now?" Like the word "is" already defines present tense. So, why do I need "now"?

Why use such an uncommon word to describe a doctor? Доктор is a cognate and much more commonly used.

To me, the sample phrase sounds like an affirmation, not like a question. How would a "real affirmation" sound like?
