my great aunt came from a conservative family and after marriage they had a weird ritual according to which looking upon a mirror post marriage could be the harbinger of ill luck. they believed that a married woman should only see her beauty through her husband's eyes.

"so my aunt has not seen herself in the a mirror"

Wait is this a real thing or just a hypothetical context where the sentence would make sense?

How would you know the difference between the reflexive and the passive voice in this sentence? Could "My aunt has not been seen in a mirror" also be a correct translation?

The passive sentence needs a personal "a":

No se ha visto a mi tía.

Perhaps that is because, for her sake, the family has removed all mirrors from the house.
