Actually, "nudzi mi się" is also a correct form, though maybe a bit colloquial.

Yes, all of them except "jestem nudzony" - such form doesn't exist in Polish

I thought Poles disliked putting 'się' at the end of a sentence.
Beginning a sentence with się is disallowed. With 'Nudzę się.' there is no other option, than to put it after the verb.
'Nudno mi.' should be an accepted answer.
I'm not sure if I've ever heard it, but it seems possible. Added now.
I'd say it's quite common and nkjp.pl seems to confirm that as well.
