doktor can also be a person with an academic qualification not related to medicine, like a doctor of philiosophy, for example. A lege is someone with a specific medical qualification. In other words all leger are doktorer but not all doktorer are leger. In my estimation the use of doktor as a synonym for lege is a bit old-fashioned nowawdays.
