Literally, maybe, but that suggests actual time being involved. Первый раз is like "first instance (of doing something)".

Yes. Pretty sure "at first time" is grammatically incorrect in English. Even if it wasn't, in Russian, you would need a preposition to match "at", such as в

Depends what you're trying to say. In English you would not say "a first time" by itself. It's usually part of an expression like "There is a first time for everything" есть в первый раз для всего or you might say "that's a first [time]" это впервые (note Russian omits the 'time' word as well)
