Noticed that some of my previously "gold" completed items are now diminishing - why is that?

Because you have to keep on refreshing an practicing to keep them gold.

They 'decay' and you can renew them by practicing.

How long does it take before the gold strength bar disappears? Is it based on the time/days since the skill was completed, or based on the words you miss (answer incorrectly) in future skills?

All of those, as well as how often you peek at the words. Generally speaking things you just learned decay more quickly than things you learned a long time ago.
