"what are you doing?" is a question in present continuous tense asking what someone is doing presently.

"what have you been doing (for last two hours /since 9 o'clock)?" is a question in present perfect continuous tense asking what someone is doing for some duration (hour(s) /day(s) /week(s) etc.) OR since a point of time in the past (9 o'clock/ morning /last Monday etc.). You need to include time aspect in this type of sentences.
