The correct response is to repeat ‘How do you do?'
It is used when you are introduced to a new person and is quite formal. The correct response is to repeat ‘How do you do?'
Note: You never use it again with that person.
The correct response is simply to repeat the question "How do you do?" or to say "Nice to meet you".
It would not be used for or by a child.
