If you look up the song "modeh ani" by Omer Adam you can see how it's used - there are subtitles in English and transliteration. There are also Spanish and French translated versions on YouTube.(The Spanish version is live in concert with Moshe Peretz and they actually start with a prayer). They are all found by searching modeh ani Omer Adam on YouTube.

How is saying אני מודֶה לךָ different from saying תודה

The phrase אני מודה לך is much more formal. If I understand correctly from your name, you are a French speaker, right? I'd say it's comparable to the French "merci" vs. "Je te/vous remercie".
