Why do we not add "aan het" to this? Isn't it an action...? I must be missing something.

Aan het is simply the present continuous which in English is 'is...ing' or 'I am... right now/currently'. So yes you can use aan het here which would be 'zijn zij ernaast aan het rennen?' -'are they currently running next to it?'
