How come it isn't "có muốn" here?

I guess the emphasis here is on "what to buy" rather than "to buy or not to buy", therefore "có" is not used in front of "muốn".

The sentence you have in mind can be used as an alternative to, for example, the following question:

"Do you want to buy something or are you just looking?"

"Buy" "mua" sounds to me like it is high rising, when it should be a level tone.

No it's definitely not a sắc tone. Even with tonal languages, there is often variation from speaker to speaker.
