Apparently "what time?" was unacceptable, but this is definitely used more often in the English that I speak.

I agree. I never include "at" before it. It doesn't seem necessary.

"At what time?" is used very often in English. For example, if someone were to say, "I'm going to the store," you would reply with, "At what time?" While you could just say, "What time?" it would be perfectly acceptable, and more correct, to say, "At what time?"

Would "at which time?" be acceptable? People don't usually say it, but it makes sense.

The answer to A que horas? is a time, possibly in hours and minutes, hours and fractions of hours or exact hours (o'clock). The question in English to get that kind of answer is At what time?.

Your question presumes that a set of hours (o'clock) has been discussed and that you want to know which one it will be.
