It's a reference to a scene that features in a couple of games in the Zelda franchise. The hand belongs to a ghost, who asks Link (the character one plays in the game) for paper.
Apparently, the inspiration came from Japanse ghost stories. I won't elaborate, in case anyone wants to sleep tonight, but here is the source.

I, a native speaker of English, would not say you are wrong but your sentence is awkward and old-fashioned.
Am i the only one here who saw this sentence and thought "Metallica"?
I have never read or heard this sentence in my life.. It is extremely awkward. Also, a horrible mareritt
Duolingo does this sometimes; provides sentences that are so bizarre they stick firmly in conciousness. Yes, they are having fun with us... but if we understand what the sentence really says, the purpose of language has been achieved... and memorably.

"Opp av" is the most natural combination for describing that the hand is coming up from the toilet bowl itself, but if it were sticking out on the side of the toilet you could use "ut av".
We also use "toalettet" to refer to the room itself, so if someone's coming "ut av toalettet" they're usually just exiting the bathroom.
