Is there a reason why "médecin" is not acceptable in this instance?

"Médecin" is his profession not his title. Similarly, a lawyer is called "Maître", a judge "Madame/Monsieur la/le Juge" (not "votre honneur"!), etc.

Is merci bien not correct here?

"merci bien" is a little less "thankful" than "merci beaucoup"

Et pourquoi pas, "M. le docteur" ?

Docteur is a title, comparable to Monsieur (any man), Maître (attorney)...

a doctor = un médecin
