Your sentences were almost correct. You can say either one, but you need to remember to use the word "to." So to edit your sentence, you would say "Would you like TO play a guitar? Or do you like TO play a guitar?"

Sorry guys...I find that second question a bit weird...I am not a native English speaker.
But the phrase:

Do you like to play a guitar?

Sounds weird, because how would you like to play a radom specific guitar?...I mean, I like to play my guitar...I like to play the guitar...I like to play any guitar....but a guitar ?....Again, I am not a native English speaker, so sorry if this is just a silly doubt...

You're right that asking someone, do you like to play a guitar, is not common. However, the sentence is grammatically correct.
