if you use the như thế nào structure, you should place it at the end of your sentence.

  • cô ấy không biết hỏi giờ bằng tiếng Việt như thế nào.
  • cô ấy không biết làm thế nào để hỏi giờ bằng tiếng Việt.
  • cô ấy không biết làm sao hỏi giờ bằng tiếng Việt.

you should leave out "để" in your sentence.

Again, that's my problem. I know "để" is usually translated 'in order to' but that does not fit in any of these sentences nor is it clear when it is needed and when it is not only not needed but wrong to include it. I'm lost on this one.

you should use "để" only with the làm sao or làm thế nào structure. loose translation: "[...] what/how to do in order to [...]". all of it need to go together.

let me give you another sentence.

tối không biết anh ấy: I don't know him.

although it's not the exact same structure, you get to see that "biết" is not followed by "để".

tôi không biết làm sao (để) làm quen anh ấy: I don't know what to do (in order) to get to know him.

it depends. have a look at the examples I gave to @okorz, only the 2nd sentence has it. to some extent, you can add "để" to the 3rd sentence right after "làm sao".
