"Jak masz na imię?" only asks about your given name, if you hear this question, you can either answer with your given name or with your full name.
'What do you call yourself' should be considered correct, since it's the literal translation.
Okay, but any person who will use "What do you call yourself" in English will draw surprised looks. This course is also used by Polish people learning English. We can't accept such a strange phrase.
It would be awesome if these translations, like asking for one's name and such, would always appear in the formal version, too. "Jak się pani nazywa?" ...because I heard bad things about being to informal too quickly in Poland, something about not having milked the cows together or such....:)
Would it be normal to use the informal you with someone whose name you don't even know? My guess (based on Spanish) is that you would in a social setting with someone of a similar age/status, but not in a business setting, but I haven't yet seen an explanation.
