愛人 usually implies a sketchy relationship in Japanese (as does "lover" in English), but IIRC this isn't the case in Mandarin -- what about in Korean?

It has no connotation in of itself. It can be "sweetheart" or "couple" or the usual "boyfriend/girlfriend" but can be used in a less positive way like you're thinking too.
