I am confused about the lack of a clear question word (like ending in 'me'). Do people just interpret sentences about their preferences as questions?

Just another learner here, but my impression is that 还是 IS a question word, in a way that "or" isn't.

You're absolutely correct: 还是 functions as the question word in this sentence.
