It is rare to hear "very extreme" or "really extreme" in English, since 'extreme' on it's own indicates something far beyond normal. To add an adjective to extreme would seem to be hyperbole to me (and my British sense of reserve!)

Also, "weather report" and "weather forecast" don't seem to be accepted here, but I don't see why either of these cannot be extreme (or even really extreme).

So right about the "very/really" extreme bit. However, it will be the weather that is extreme, not the weather forecast/report.
