In British English at least, nobody would say 'that mister'. While some dialects do address unknown men as 'mister' ("Oi, mister! You forgot your coat!"), even those speakers would say 'do you know that gentleman'.

'Sir' is a formal way of addressing someone in English if you don't know his name. No native English speaker I've ever met would say 'that sir' when they meant 'that gentleman'.

Why not, "That guy, do you know him?" Like you are speaking to a friend.
