Translation:Señor Albert, ¿cómo está usted?
¿Cómo estás? is the familiar way to ask, for friends, family, children.
¿Cómo está (usted)? is the formal way, for strangers, people you aren't on a first-name basis with, authority figures.
Dúo has used the formal here because we're addressing the gentleman as "Señor."
