No, not necessarily (I think).
Kakak = older brother/sister.

This person is older, not necessarily bigger/taller.
I don't know if "big brother" and "big sister" have the same meaning in English.

'big brother/ sister' should be accepted for 'kakak'. they are synonymous with 'older brother/ sister'.

similarly, 'little brother/ sister' should be accepted for 'adik' or 'younger brother/ sister'.

'big' and 'little' in the context of siblings refers to relative age in english.
