Is "serta" more formal than "dan", or when is it used?

No, its the same in this context. Serta sometime can be translated as "with" > Andi beserta keluarganya. Andi serta keluarganya.

Serta is not more formal. Actually in this context, "serta" and "dan" is the same. But "beserta" is a better translation. Beserta/serta has "with" nuance.

Serta is just alternative of diction "dan" for writing/speaking. It can used to make a text not too boring with many "dan"

Why is "dan" not allowed?

It is. Serta and dan is interchangeable in this sentence.

Actually 'dan' is correct. The sentence can also :

"Tono dan orang tuanya tidak makan wortel."

Why is wortel wortel not accepted

Becz to write "wortel" only in this case, it has already representated all of carrots or already plural

"serta" is kinda weird, more people use "beserta" instead. Greeting from an Indonesian living in Saudi Arabia.
