No guarantee that this statement refers to an older sister. He could be an older brother, too. The sex of the older sibling hasn't been specified. /Kakak/ alone doesn't tell the reader whether the person is male or female. You'd have to have either /kakak laki-laki/ or /kakak perempuan/ to know for certain.
