Is this teaching us to use more polite speech to those working in the medical field?
The formal 'vous' should be used for addressing anyone (not only doctors) who is not a close friend or relative. It would be considered rude if you address a stranger with "tu".
In France, yes, but in Québec the informal "tu" is perfectly acceptable-at least thst has been my experience.
What if you were talking to your brother or sister - you wouldn't use "vous" would you?
Nope, "vous" is the formal 'you'. To a family member or close friend you would use "tu": "Je suis sûre que tu aurais été un très bon médecin."
