Is L' used when the noun starts with a vowel, as a rule?

Almost. For male nouns, "Lo" is always used before vowels, as a rule, and it is truncated to L' before most vowels; however, before a semiconsonantic /j/ sound it doesn't get truncated: "lo iodio", "lo yogurt".
