When is the "an" abreviated to " a' " abd why?

There is only one a' so can only sensibly write His sister and his/the mother.

I keep forgetting to add the ' after the a but it keeps getting marked correct and unless I go to the discussion page I don't find out I'm wrong.

How does one differentiate between "a phiuthar" and "a' phiuthar"? I am guessing you would need to know the context of the conversation?
