Sometimes "mesmo que" is translated as "even if" and other times it's "even though". Are both correct translations?

Yup. "Even though" can be also translated as "mesmo assim, apesar de"

Can you use the subjunctive after "mesmo assim"?

No! It works to contrast thoughts.
