Beyond the simple difference between leur + singular noun & leurs + plural noun; you should write:

"Les éléphants étirent leurs pattes" (The elephants stretch their legs) but "Les éléphants secouent leur trompe" (The elephants shake their trumps) - because each elephant has several legs, but only one trump.

By the way, "Leur" can be a pronoun, as in "Il leur parle" (He talks to them), in which case it is always singular.

Yes, because it's a possessive adjective and agrees with the noun it describes, like mon/ma/mes, ton/ta/tes ...
