How should I think of 'Entonces' in the structure of this question when translating to English? Would this even be used in every day Spanish?

I'm pretty sure that commission doesn't need 'a'.

Both with it and without it are accurate, like how "she received payment" and "she received a payment" are both accurate.

You are thinking of commission paid, this is referring to a commission assigned I believe.
