Interesting. So "お姉さん" is formal and "姉" is informal? That would make sense, with the "お" prefix and the "さん" suffix. Would this work with other family members?

Both are formal, but should be distinguished.

お姉さん: This is an honorific expression. As you pointed out, it is used to mention somebody outside one's family or relative.

姉: Humble expression, used exclusively to mention somebody in one's family or relative, to other people. (Do NOT use for someone of other family members!)

For example:

His sister is a high school student 彼のお姉さんは高校生です

My sister is a high school student 私の姉は高校生です

The following pairs are similar examples (honorific ⇔ humble).
