Is there a less cumbersome way to say "The doctor shakes his head"?

There isn't really actually. You could also say 'Fez que não com a cabeça' or 'Disse que não com a cabeça' or variations of them

Why is this not "with the head"? Alternatively, why don't we need "dele" at the end of the sentence?

Was tempted to write 'The doctor shakes his head', but went the literal route. Did 'shakes his head' pass?
