I wrote "Wat ben ik verantwoordelijk voor" which was marked as incorrect. Shouldn't that be OK?

no, it is not correct. it should be 'voor wat ben ik verantwoordelijk' or 'waar ben ik verantwoordelijk voor'

One of Duo's translations of "For what am I responsible?" is "Waar ben ik verantwoordelijk voor?" I don't understand the usage of "waar" in this context.

It's waarvoor, which means 'what for'. The voor separates from the waar and comes at the end of the phrase (in this case so does the English translation), so 'What am I responsible for?' translates as 'Waar ben ik verantwoordelijk voor?'
