I never went to England...shouldn't that be accepted as well?

Wouldn't it be more accurately, 'I have never gone to England'? With 'Non sono mai stato in Inghilterra' being 'I have never been to England'?

"I have never gone to England" is accepted.

How flexible is "mai"? Could it be, "Non sono andato mai in..."?

I never have been to England was wrong. Is the word order too stilted? The exercise was with tiles where been was the only verb choice.
