As far as I know

verjaardag= birthday

geboortedatum= date of birth / birth date

I wrote birth date on February 15-th 2015 and it was correct. I don't know if date of birth is wrong but birth date feels more natural to me.

That's interesting.. on all official documents I've seen, it's always 'date of birth' and never 'birth date', although both are correct.

That what you said would confirm my thought in a way. As you stated, you've seen date of birth on documents and maybe that created the "unnatural" impression in me. Nevertheless, both versions are correct and should be accepted.
