The implication here is that he will very shortly receive his qualification as a doctor. There are such subtle differences between the two languages. This sentence from Duo certainly does not mean that he is incompetent.

Hi Lahure! I was sarcastic about a brazilian government program. So... In portuguese, it's common to use this sentence to express the incompetence of a professional.

Maybe he is part of a sort of government program that pay the double to another country to send doctors receiving less than half to their colleagues. These doctors can't leave the country nor bring their families to live with them.
