It's not a coincidence. Darth = dark + Sith or dark + death (English, of course); Vader was purposely named from the Dutch word for father as foreshadowing.

Actually no. George Lucas just thought Vader sounded ominous when he came across the word. They only came up with the idea to make Vader Luke's father when they were making The Empire Strikes Back.

It does work, but we only use 'u' or in this case 'uw' if it's strictly formal situation. So it is a correct translation (and answer), but seen the context and the formal use of 'u' in dutch, it's not the 'best' answer.
