Well, it has happened to me a couple of times. Although, it happens randomly. I think it often happens when the network is unstable, and there are connectivity problems. The issue is that in that case I could and did continue the lesson till the end, yet it was confusing, as it allowed me to continue, although I had not answered the question.

I've had a similar experience with unstable network connections. I can refresh the page using Smooth Gestures 0.17.7 for Chromium (Version 28.0.1500.71 Ubuntu 13.04 (28.0.1500.71-0ubuntu1.13.04.1), but before it's refreshed, the "Continue"-button is there, and I can use it.

Iirc, this doesn't give me points or advance the green bar though.

Maybe, that's the formula for replicating the bug. Unstable connections + Linux operating systems = Bug with toolbar and continue button.

Something similar happens to me when there's an electricity cut (=Internet cut) showing me that my answer is correct whatever I write, and allowing me to continue answering the questions (or even leaving them blank), so if the Internet connection is back before finishing, I can complete the lesson 'successfully' i.e. it will be registered as learned.
