Why is it "what fell" and not "what did fall"?
Because that's how you ask that question in English.
'fell' is the past form of 'fall'
You wouldn't say 'did fall' normally. It's the emphatic way to say 'what fell'. Also, when used non-emphatically, it indicates that 'what' refers to the object of the subsequent clause.

From the audio it is hard to distinguish, whether the speaker says "Kio falis?" or "Kiu falis?" - it should be a bit slower maybe...
Is "who fell" right?
That would be "Kiu falis?"
Is "what did fall" acceptable?
