Why is acı is unacceptable? Whats the difference from ağrı?

"acı" is a short, sharp pain as opposed to something more chronic, which is "ağrı". It would be kind of strange in this context.

I remember seeing that to say you're in pain, you can use can plus ağrımak or acımak. Would it be okay to say something like "Doktor, canım çok ağriyor."? Or does the can make it more of a psychological kind of pain?

"Can" is only used with "acımak". It's only used when the location of the pain has already been established. So you can't exactly go to the doctor and say "canım acıyor". You first tell them which part of your body is acıyor. Afterwards, when the doctor is treating you, if what s/he does really hurts you, you can say "öyle yapınca canım acıyor" (It hurts when you do that).

"Canım ağrıyor" is not idiomatic. :)

is 'doktor bey' acceptable and what about 'çok ağrıyorum'?

"Doktor Bey" for men and "Doktor Hanım" for women can be used.

And "Çok ağrıyorum" is not acceptable; for a body part we can use "(Buram/şuram) ağrıyor", for whole body we can use "Her yanım (yerim) ağrıyor"
